Case Management

The Case Management Service was initiated for members of Muscular Dystrophy NSW requiring assistance and support.  This is a client centered service which works alongside members to address and support individuals to reach identified personal goals.

Case Management can also be used to provide somebody neutral to talk to; linking members with services in the community; assistance in planning a holiday, and support around identifying day time activities or employment.  As well as providing information about services and programs available to members.  Individual Face to Face contact also enables members and Muscular Dystrophy NSW to develop closer connections.
